This Physical Therapy Initial Assessment is a time to meet with your physical therapist to discuss your concerns and begin identifying contributing factors. We will establish goals for physical therapy, provide an initial home exercise program, and map out next steps in your care plan.

This time is used to reassess and progress your physical therapy program. Sessions are tailored to your unique goals and needs identified during your initial evaluation. You will collaborate with your therapist to update your home program and self care plan.

Video visits allow you to connect with your therapist and adapt your program without the commute to the clinic. Video visits may be used to supplement your program. These visits are appropriate for individuals with good symptom control and independent mobility who are looking for expert advice on how to manage their condition with various movement techniques, exercise, and mobility drills that can be done at home.

In-home/On-site visits are available on a case by-case basis and are typically reserved for individuals who are too dizzy to drive to their initial visit.
Our Process
Insurance
As an out-of-network provider I do not accept or communicate directly with insurance providers. Payment is due at time of service and you are responsible for payment. Superbills will be provided for you to submit to insurance for possible reimbursement. Unfortunately, reimbursement varies significantly and cannot be garunteed.
Medicare
I am unable to accept Medicare cases and services are not eligible for reimbursement from Medicare. Due to Medicare law, I am unable to provide Physical Therapy services to Medicare recipients. However, certain health and wellness services are available to Medicare recipients on a cash-pay basis. In my practice, this includes balance assessment and vestibular assessment for overall health and performance, particularly related to goals of sport and recreation.
Working together
Every case is different, yet there are some common themes. I provide expert guidance on how to self manage your condition. My goal is to help you feel more confident in your physical therapy plan so you can carry it out day to day. During your evaluation we will decide whether you are ready to work on your physical therapy plan independently with follow up as needed, or whether your case will need a series of weekly or bi-weekly visits to establish your routine. Because many of the people I work with have lifelong conditions, you may benefit from long term follow up either monthly, quarterly, or yearly.
Creating a plan
In addition to your detailed PT assessment, I will provide you with detailed information for your home exercise plan and strategies to complete your day to day activities while navigating your health. Whether you are recovering from a concussion, working on your balance after an MS flare, or working through a case of BPPV, I will help you create a plan to move forward.
